We address and help solve the issues that keep patients from managing their own care effectively, often leading to readmissions. Our teams are trained in motivational interviewing aligned with HCAHPS 3-Item Care Transition questions to increase scores and ensure that patients understand all aspects of their care.
Following Patients Until Day 31 Post Discharge
Working as an extension of hospital teams, our connection with each patient starts before discharge, through any post-acute care transition, and continues until day 31 after discharge. Multiple touch points of personal contact — not software — guide patients through their care journey, creating a seamless safety net that helps to avoid readmissions and close care gaps.
