Post-Discharge Care
365Telecare’s connection with patients begins before discharge. Using motivational interviewing, our teams build trust and connect with the patient during the care transition and at regular touchpoints during and beyond the first 30 critical days. Care teams work with patients on areas including self-care management, medication management, and disease-specific issues, and identify and mitigate any issues that might lead to readmission.
Care Gap Closure
Teams of clinically trained staff mobilize to close a high volume of care gaps to improve quality scores. Experienced in contacting hard-to-reach populations, care teams case manage each identified patient to close critical care gaps. Teams ensure that claim information makes it through the system efficiently and effectively in order to increase gain sharing and reimbursements.

Are you a software platform?
Our teams of trained and experienced caregivers work to make personal connections with patients through phone, video calls, and/or text on a regular basis in order to prevent readmissions and close care gaps. Using our custom-built software, our teams stay organized and engaged in well researched, pre-scheduled contact points which have been proven to lead to long-term success in preventing hospital readmissions.
